Alendronate

Alendronate is used to treat and prevent postmenopausal and steroid-induced osteoporosis, it stimulates formation of the bones, increases their mineral density.

Common Use

Alendronate belongs to a class of medications called bisphosphonates—in simple terms, these are drugs that help strengthen bones by slowing down the natural process of bone breakdown. It is used to treat and prevent several conditions related to bone loss:

Osteoporosis in postmenopausal women – a condition where bones become weak and brittle after menopause, increasing the risk of fractures

Osteoporosis in men – age-related bone thinning that can also affect men, especially as they get older

Steroid-induced bone loss – bone thinning caused by long-term use of corticosteroid medications (such as prednisone), which can weaken bones over time

Paget’s disease of bone – a condition where bone renewal happens too quickly, leading to weak, misshapen bones (in some formulations)

Alendronate works by slowing down the cells that break down old bone, giving your body more time to rebuild new, stronger bone tissue. This helps increase bone mass and reduces the risk of fractures, especially in the spine (vertebrae) and hips, which are common sites for serious fractures in people with osteoporosis. It is typically prescribed for long-term bone health maintenance.

Dosage and Direction

Alendronate is taken by mouth, either once daily or once weekly, depending on your condition and the specific dosing form your doctor prescribes. The instructions for taking it are very important—following them carefully helps prevent irritation to the food pipe (esophagus). Here’s what you need to do:

Take it first thing in the morning, before you have anything to eat or drink

Swallow the tablet with a full glass of plain water (not tea, coffee, juice, or mineral water)

Do not chew, crush, or suck on the tablet

After taking it, remain upright—sitting or standing—for at least 30 minutes. Do not lie down, as this can cause the medication to travel back up into the esophagus

Wait at least 30 minutes before eating, drinking, or taking any other medications

Precautions

Before taking alendronate, let your doctor know if you:

Have problems with your esophagus (food pipe), such as difficulty swallowing or conditions like Barrett’s esophagus

Have low calcium levels (hypocalcemia) – this needs to be corrected before starting treatment

Have kidney problems or severe kidney impairment

Cannot sit or stand upright for at least 30 minutes

While on alendronate, make sure you are getting enough calcium and vitamin D, either through your diet or supplements—your doctor will advise you on this. Regular dental check-ups are recommended, as in very rare cases, jaw bone problems (osteonecrosis of the jaw) have been reported, particularly after dental procedures.

Contraindications

Alendronate is contraindicated—meaning you should not take it—in the following situations:

Abnormalities of the esophagus that delay emptying (such as strictures or achalasia)

Inability to sit or stand upright for at least 30 minutes

Hypocalcemia (low blood calcium) that has not been corrected

Severe kidney impairment

Known allergy to alendronate or any of its components

Possible Side Effects

Common side effects may include:

Stomach pain or discomfort

Acid reflux (heartburn)

Constipation or diarrhoea

Muscle or joint pain

More serious side effects, though less common, include:

Esophageal irritation, ulcers, or difficulty swallowing – this is why following the dosing instructions is so important

Rarely, osteonecrosis of the jaw (bone damage in the jaw) – symptoms include pain, swelling, or infection in the jaw area

Unusual thigh bone fractures (atypical femoral fractures) – if you develop new or unusual pain in your hip or thigh, let your doctor know

Contact your doctor if you develop chest pain, severe bone or joint pain, or any discomfort in your jaw.

Drug Interactions

Alendronate should not be taken at the same time as:

Calcium supplements

Antacids (for heartburn or indigestion)

Other oral medications

These can interfere with the absorption of alendronate, making it less effective. You need to wait at least 30 minutes after taking alendronate before consuming anything else. Some medications for stomach acid, such as proton pump inhibitors (PPIs) , may also affect absorption or increase the risk of side effects. Always let your doctor know about all medications and supplements you are taking.

Missed Dose

If you take daily alendronate and miss a dose: skip it and resume your regular schedule the next day. Do not take two doses on the same day.

If you take weekly alendronate and miss a dose: take it the next morning after you remember. Then return to your original weekly schedule. Do not take two doses in the same day.

Overdose

Overdose of alendronate may cause:

Low blood calcium or low blood phosphate levels (which can cause muscle cramps or numbness)

Serious digestive tract irritation (stomach or esophageal pain)

If you suspect an overdose, seek medical attention immediately. Do not induce vomiting, and do not lie down after taking an extra dose.

Storage

Store alendronate at room temperature, away from heat and moisture. Keep the tablets in their original container, tightly closed, and out of reach of children and pets. Do not use expired medication. When you no longer need it, dispose of unused medicine as instructed by your pharmacist.
