Here’s how it works in plain language: Calcium is a mineral that triggers the contraction (tightening) of smooth muscles, including those in the walls of arteries and the heart itself. Arpamyl blocks calcium from entering certain cells, which causes:
Relaxation of arterial muscles – blood vessels widen (vasodilation)
Improved oxygen delivery to the heart muscle (myocardium)
Decreased total peripheral resistance – meaning the heart doesn’t have to work as hard to push blood through the body
Lower blood pressure (reduced systemic blood pressure and afterload)

Precautions

Contraindications

Atrial flutter or atrial fibrillation with an accessory bypass tract (such as Wolff-Parkinson-White syndrome or Lown-Ganong-Levine syndrome)
Possible Side Effects
Arpamyl is usually well tolerated, but side effects can occur.
Most common side effects may include:
Dizziness
Weakness or fainting
Swelling of the lower extremities (edema) – particularly in the ankles and feet
Rash
Headache
Constipation – this is a common side effect with verapamil
Less common side effects may include:
Jaundice (yellowing of the skin or eyes)
Swelling (fluid retention)
Difficulty breathing
Rashes

Drug Interactions

Cyclophosphamide, vincristine, procarbazine, prednisone (COPP) and vindesine, adriamycin, cisplatin (VAC) – these cytotoxic drug regimens may decrease absorption of Arpamyl
Aspirin – co-administration may increase the risk of bleeding in rare cases
Digoxin – Arpamyl can increase serum digoxin levels by 50% to 75% during the first week of therapy, which can lead to digitalis toxicity (serious heart rhythm problems). Correction of digoxin dosage is essential
Oral antihypertensive agents – taking them together can result in additive blood-pressure-lowering effects
Disopyramide, flecainide, quinidine – may increase the risk of heart rhythm problems
Phenobarbital – may reduce the effectiveness of Arpamyl
Cyclosporin – Arpamyl may increase cyclosporin levels
Lithium – Arpamyl may increase lithium levels, leading to toxicity
Arpamyl may also cause mildly abnormal results on certain liver function tests—your doctor will monitor this.
Missed Dose
If you forget to take a dose, take it as soon as you remember—unless it is almost time for your next dose. If that’s the case, skip the missed dose and return to your regular schedule. Do not take a double dose to make up for a missed one.
Overdose
If you experience slow heartbeat and fainting, these may be symptoms of an Arpamyl overdose. If you suspect that too much of the medication has been taken, seek immediate medical attention.
Storage
Store Arpamyl at room temperature between 15–30°C (59–86°F), away from light, moisture, and out of reach of children and pets.
