Amiodarone

Amiodarone is used to correct abnormal rhythms of the heart, to treat potentially life-threatening types of abnormal heart rhythms.

Common Use

Amiodarone is an antiarrhythmic medication—in simple terms, it helps stabilize the heart’s electrical activity and maintain a regular heartbeat. It is used to treat and prevent serious, potentially life-threatening heart rhythm disorders, including:

Ventricular tachycardia – a dangerously fast heart rhythm that starts in the lower chambers of the heart (ventricles)

Ventricular fibrillation – a chaotic, irregular heart rhythm that causes the heart to stop pumping blood effectively and can lead to sudden cardiac arrest if not treated immediately

Amiodarone is usually prescribed when other antiarrhythmic medications have failed or are not suitable. Because it can affect multiple organs over time, it is typically used under close medical supervision, especially for long-term treatment.

Dosage and Direction

Amiodarone can be taken:

Orally (as tablets) for long-term management, or

Intravenously (through a vein) in a hospital setting for acute arrhythmias that require immediate control

Treatment usually starts with a loading dose—a higher dose for the first few weeks to get the medication into your system—followed by a maintenance dose that is lower and taken long-term. Your doctor will tailor the dose based on your condition, how well you respond, and your lab results.

Always take amiodarone exactly as prescribed, and never stop taking it suddenly without medical advice, as this can trigger dangerous heart rhythm disturbances.

Precautions

Before starting amiodarone, be sure to tell your doctor if you have:

Lung disease (such as asthma, COPD, or pulmonary fibrosis)

Liver problems

Thyroid disorders (overactive or underactive thyroid)

Vision issues

Amiodarone can affect multiple organs, so regular monitoring is essential. Your doctor will arrange periodic tests to check your:

Lung function – to watch for signs of lung toxicity

Liver function – through blood tests

Thyroid function – because amiodarone contains iodine and can affect thyroid hormones

Eye function – to monitor for deposits in the cornea that can affect vision

Avoid grapefruit juice while taking amiodarone, as it can increase the concentration of the medication in your body, raising the risk of side effects. Protect your skin from sunlight, as amiodarone can cause photosensitivity—making your skin more sensitive to UV rays and more likely to burn or develop a blue-gray tint with long-term use.

Contraindications

Amiodarone is contraindicated—meaning you should not take it—in the following situations:

Severe sinus node dysfunction (problems with the heart’s natural pacemaker)

Bradycardia (very slow heart rate) without a pacemaker

Second- or third-degree AV block (a delay or block in the electrical signal from the upper to lower chambers of the heart) without a pacemaker

Known hypersensitivity (allergy) to iodine or amiodarone

Cardiogenic shock (a condition where the heart suddenly cannot pump enough blood to meet the body’s needs)

Use during pregnancy is not recommended unless absolutely necessary, as it can affect the baby’s thyroid function.

Possible Side Effects

Common side effects may include:

Nausea

Fatigue (tiredness)

Tremors (shaking, especially in the hands)

Changes in taste

Long-term use can lead to more serious complications, which is why regular monitoring is so important:

Lung toxicity – scarring or inflammation of the lung tissue, which can cause cough and shortness of breath

Liver dysfunction – which may show as yellowing of the skin or eyes (jaundice), dark urine, or abdominal pain

Thyroid imbalance – either overactive thyroid (hyperthyroidism) or underactive thyroid (hypothyroidism)

Vision problems – including corneal deposits that can cause halos or blurred vision, and rarely, optic nerve damage

Skin discoloration – a blue-gray tint to the skin, usually after long-term use

Report any breathing difficulty, yellowing of the skin or eyes, or vision changes to your doctor immediately.

Drug Interactions

Amiodarone interacts with many medications. It is particularly important to be aware of interactions with:

Warfarin and other blood thinners – amiodarone can increase the risk of bleeding and requires careful monitoring of INR levels

Digoxin – levels may rise to toxic levels

Statins (cholesterol-lowering medications) – amiodarone can increase the risk of muscle damage

Other antiarrhythmic medications – may increase the risk of serious heart rhythm problems

Beta-blockers and calcium channel blockers – can cause excessively slow heart rate

Certain antibiotics, antidepressants, and antifungals – may affect amiodarone levels

Always inform your healthcare provider of all medications and supplements you are taking, including over-the-counter products and herbal remedies.

Missed Dose

If you miss a dose, take it as soon as you remember—unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular schedule. Do not take two doses at once to make up for a missed one. Missing doses may reduce the effectiveness of the medication in controlling your heart rhythm.

Overdose

An overdose of amiodarone may cause serious heart problems, including:

Very slow heartbeat (bradycardia)

Fainting

Cardiac arrest

If you suspect an overdose, seek emergency medical attention immediately. Treatment involves monitoring and supportive care in a hospital setting. Never take more than the prescribed dose.

Storage

Store amiodarone at room temperature, away from heat, light, and moisture. Keep it in its original container and out of reach of children. Do not use the medication after the expiration date. Ask your pharmacist how to dispose of unused medicine properly.
