Abiraterone

Abiraterone is a hormonal therapy used to treat advanced prostate cancer by blocking testosterone production. It is taken orally with prednisone and requires regular monitoring due to potential effects on the liver, blood pressure, and electrolytes.

Common use

Abiraterone is a tablet you take by mouth for prostate cancer — specifically a type called metastatic castration-resistant prostate cancer (mCRPC) . Let’s break that down: metastatic means the cancer has spread beyond the prostate, castration-resistant means it keeps growing even when testosterone levels are very low. In Cyprus, where we have many pappoudes (grandfathers) going through this, Abiraterone is one of the treatments doctors turn to when the cancer is advanced.

It works by blocking an enzyme called CYP17 — think of it as turning off a tap that makes androgens (male hormones like testosterone). Since prostate cancer feeds on testosterone, lowering it helps slow things down. Abiraterone is always given together with a corticosteroid like prednisone — they’re like a pair of kalamari and skordalia, they work best together.

Dosage and direction

You take this once a day, on an empty stomach — that means at least one hour before eating or two hours after. Don’t take it with food, because food makes your body absorb too much too fast, and that can cause trouble. You’ll also be taking prednisone alongside it to protect your adrenal glands and keep side effects in check.

Your oncologist will give you the exact dose that suits you. Stick to it like glue — don’t change it without having a word with your doctor first.

Precautions

Before you start, tell your doctor if you’ve ever had heart problems, liver issues, or high blood pressure (what we call piési in Cyprus). This medication needs watching — your doctor will check your blood pressure, liver enzymes, and potassium levels regularly to make sure everything is in order.

If you’re a woman, especially if you’re pregnant or could become pregnant, don’t handle crushed or broken tablets — the medication can harm a developing baby. Men taking it should also use contraception during treatment and for a while after.

Stay hydrated — especially in the Cypriot heat — and if your doctor puts you on a low-sodium, low-fat diet, follow it. Your body will thank you.

Contraindications

This medication is not for people with severe liver problems, or anyone allergic to abiraterone acetate or any of the ingredients in the tablet. It’s not approved for women or children. It should not be taken with certain strong medications that affect liver enzymes, and again — never take it with food.

Possible side effect

The common ones you might feel: joint pain (like your gonata are complaining), fluid retention (swelling in the legs or feet), high blood pressure, and low potassium (which can make you feel weak or crampy).

Liver function changes are common too — that’s why you’ll get regular blood tests. Fatigue, hot flushes (yes, men can get them too), and urinary tract infections can also happen.

Rarer but more serious: adrenal insufficiency (when your body doesn’t make enough of certain hormones), heart rhythm problems, or liver toxicity. If you feel unwell in a way you can’t explain, don’t brush it off — call your doctor.

Drug interaction

Abiraterone can interact with a number of other medications. It affects how the liver processes certain drugs, and other drugs can affect how it works. Be especially careful with blood thinners like warfarin, some antidepressants, and antiepileptics. Strong drugs that speed up liver enzymes — like rifampin or phenytoin — can make abiraterone less effective.

Give your doctor a full list of everything you’re taking — including anything from the pharmacy, herbal bits, and even supplements you might have picked up at the laiki (farmers’ market). Better to be upfront than sorry.

Missed dose

If you forget a dose, take it as soon as you remember on the same day. But if a whole day has passed, skip it — don’t double up. Just take your next dose at the usual time the next day. Keeping a regular schedule helps things run smoothly.

Overdose

If you think you’ve taken too much — whether by accident or mistake — get medical help straight away. Overdose can cause high blood pressure, low potassium, or liver problems. There’s no magic antidote, so doctors will treat the symptoms. Never take more than the prescribed dose.

Storage

Keep your tablets at room temperature, away from moisture and heat. Don’t leave them in the bathroom — too much humidity. Keep them in the original container with the lid tightly closed, and somewhere kids and pets can’t reach. When they’re out of date, don’t just throw them in the bin — ask your pharmacist how to dispose of them properly.
