Azilsartan and Chlorthalidone

Azilsartan and chlorthalidone is a combination medication used to treat high blood pressure. It contains azilsartan, which helps relax blood vessels, and chlorthalidone, a diuretic that helps remove excess salt and water. This dual action effectively lowers blood pressure and reduces cardiovascular risks. It is typically taken once a day and requires regular monitoring.

Common Use

Azilsartan belongs to a class of medications called angiotensin II receptor blockers (ARBs) —in simple terms, it works by blocking the action of a hormone called angiotensin II. This hormone normally causes blood vessels to narrow (constrict) and signals the body to retain salt and water. By blocking angiotensin II, azilsartan helps:

Relax and widen blood vessels – allowing blood to flow more easily

Reduce fluid retention – helping to lower blood pressure

Decrease the workload on the heart

Azilsartan is used to treat hypertension (high blood pressure)—a condition where the force of blood against the artery walls is consistently too high. By lowering blood pressure, it helps reduce the risk of strokes, heart attacks, and other cardiovascular complications.

Azilsartan is sometimes combined with another medication called chlorthalidone (a diuretic/water pill) for more effective blood pressure control.

Dosage and Direction

Azilsartan is usually taken once daily, with or without food—whatever suits you best. Your doctor will determine the appropriate dose based on your response to treatment and your medical condition.

The typical starting dose is often 40 mg once daily, and your doctor may adjust the dose up to 80 mg once daily if needed.

Take the medication at the same time each day to help maintain consistent levels in your body. Follow your doctor’s instructions carefully—do not change the dose or stop taking it without consulting your doctor first.

Precautions

Before taking azilsartan, inform your healthcare provider if you have:

Kidney problems (renal impairment)

Liver problems (hepatic impairment)

Heart failure

Diabetes

Low sodium levels (hyponatremia)

High potassium levels (hyperkalemia)

Volume depletion – such as from severe vomiting, diarrhea, or taking high doses of diuretics (water pills)

You are on a low-sodium diet

Important:

Avoid becoming dehydrated or overheated – excessive sweating, severe diarrhea, or vomiting can cause fluid loss and may increase your risk of side effects such as low blood pressure (dizziness, fainting)

Pregnancy warning: Azilsartan can cause serious harm to an unborn baby and should not be used during pregnancy. If you become pregnant while taking this medication, stop it immediately and contact your doctor. Women of childbearing age should use effective birth control

Contraindications

Azilsartan is contraindicated—meaning you should not take it—in the following situations:

Allergy to azilsartan, chlorthalidone (if taking the combination), or any of its components

Pregnancy – especially during the second and third trimesters

Anuria – the inability to produce urine (complete kidney failure)

Possible Side Effects

Common side effects may include:

Dizziness or lightheadedness – especially when standing up quickly (this is most common in the first few days of treatment)

Fatigue (tiredness)

Increased urination (if taking the combination with chlorthalidone)

Diarrhea or nausea

Headache

Serious side effects (rare but require medical attention) may include:

Electrolyte imbalances – such as low sodium (hyponatremia) or high potassium (hyperkalemia). Signs include muscle weakness, irregular heartbeat, confusion, or severe fatigue

Kidney dysfunction – changes in urination, swelling in the legs or ankles

Severe allergic reaction – rash, hives, swelling of the face or throat, difficulty breathing

Low blood pressure (hypotension) – severe dizziness or fainting

If you experience any serious side effects, contact your doctor immediately.

Drug Interactions

Azilsartan can interact with several medications. Always inform your doctor about all medicines and supplements you are taking, including over-the-counter products and herbal remedies.

Important interactions:

Lithium – azilsartan may increase lithium levels in the blood, which can lead to lithium toxicity

Non-steroidal anti-inflammatory drugs (NSAIDs) – such as ibuprofen, naproxen, or celecoxib—may reduce the blood-pressure-lowering effect of azilsartan and increase the risk of kidney problems

Other blood pressure medications – taking them together may increase the blood-pressure-lowering effect and cause dizziness or fainting

Potassium-sparing diuretics (such as spironolactone) and potassium supplements – may increase potassium levels in the blood (hyperkalemia)

Diuretics (water pills) – may increase the risk of dehydration and low blood pressure

Missed Dose

If you miss a dose, take it as soon as you remember—unless it is almost time for your next dose. If that’s the case, skip the missed dose and continue with your regular schedule. Do not double up to make up for a missed one.

Overdose

In case of overdose, seek immediate medical attention. Symptoms may include:

Severe dizziness or fainting (due to very low blood pressure)

Electrolyte imbalance

Slow or irregular heartbeat

Storage

Store azilsartan at room temperature, away from moisture and heat. Keep it out of reach of children and pets.
