Atacand is an antagonist of vasoconstrictor angiotensin II. The drug relaxes and widens blood vessels to treat hypertension.
Common Use
Atacand contains the active ingredient candesartan cilexetil, which belongs to a class of medications called angiotensin II receptor blockers (ARBs) —in simple terms, it works by blocking the action of a hormone called angiotensin II. Angiotensin II normally causes blood vessels to narrow (constrict) and signals the body to retain salt and water. By blocking this hormone, Atacand helps:
Relax and widen blood vessels
Lower blood pressure
Reduce the workload on the heart
Improve blood flow
Atacand is used to treat:
Hypertension (high blood pressure) – by lowering blood pressure, it reduces the risk of stroke, heart attack, and kidney problems
Heart failure – to help improve symptoms and reduce hospitalizations in patients with heart failure
Impaired left ventricle systolic function – a condition where the heart’s main pumping chamber (the left ventricle) does not contract effectively, often following a heart attack
Dosage and Direction
Take Atacand with or without food—whatever suits you best. Take the medication exactly as prescribed by your doctor, as different conditions require different doses.
Condition Starting Dose Maximum Dose Congestive heart failure 4 mg once daily Up to 8 mg once daily (for mild to moderate heart failure) Hypertension (high blood pressure) 8 mg once daily Up to 16 mg once daily Your doctor may adjust your dose gradually based on your response to treatment and how well you tolerate the medication.
Precautions
Before taking Atacand, inform your doctor if you have:
Heart failure – your dose may need to be adjusted carefully
Renal impairment (kidney problems) – your kidney function may need to be monitored
Renal artery stenosis – narrowing of the arteries that supply blood to the kidneys
Aortic or mitral valve stenosis – narrowing of the heart valves
Obstructive hypertrophic cardiomyopathy – a condition where the heart muscle becomes abnormally thick
Hypertension with intravascular volume depletion – low blood volume (for example, after taking high doses of diuretics or after severe diarrhea or vomiting)
Hyperaldosteronism – a condition where the adrenal glands produce too much of the hormone aldosterone
If you are going to have surgery – your blood pressure may need to be monitored more closely
Contraindications
Atacand is contraindicated—meaning you should not take it—in patients with hypersensitivity (allergy) to candesartan or any of the components of the drug.
Possible Side Effects
The most common side effects reported with Atacand may include:
Anemia (low red blood cell count)
Bloody stools
Mental confusion
Cerebral infarction (stroke)
Fatigue (tiredness)
Nausea
Renal failure (kidney failure)
Malaise (general feeling of being unwell)
Dizziness
Loss of consciousness (fainting)
Atrial fibrillation (irregular heartbeat)
Chest pain
Important: Adverse events, especially renal impairment (kidney problems) and hyperkalemia (high potassium levels), may increase when Atacand is co-administered with an angiotensin-converting enzyme (ACE) inhibitor (another class of blood pressure medication). Your doctor will monitor you closely if you are taking these medications together.
Drug Interactions
Atacand has been studied with several medications and no significant interactions have been reported when used with:
Glyburide – a diabetes medication
Warfarin – a blood thinner
Nifedipine – a calcium channel blocker
Digoxin – a heart medication
Hydrochlorothiazide – a diuretic (water pill)
Oral contraceptives – birth control pills
Enalapril – an ACE inhibitor (used together in heart failure patients)
Important interactions to be aware of:
Lithium – blood levels of lithium may increase when Atacand is taken with medications containing lithium. Your doctor should monitor lithium levels
Potassium-sparing diuretics (such as spironolactone), potassium supplements, and salt substitutes containing potassium – taking these with Atacand may increase potassium levels in the blood (hyperkalemia), which can be dangerous. Always inform your doctor if you are using any of these
Interactions with drugs metabolized by the cytochrome P450 system are not expected because Atacand is not significantly metabolized by this pathway.
Missed Dose
Never take a double dose of this medication. If you miss a dose and it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not take extra to make up for a missed dose.
Overdose
Symptoms of Atacand overdose may include:
Hypotension (very low blood pressure)
Dizziness or lightheadedness
If an overdose is suspected, seek immediate medical attention.
Storage
Store Atacand at a temperature not exceeding 30°C (86°F). Keep it away from moisture, heat, and direct sunlight. Keep out of reach of children and pets.
