Amiloride is a potent diuretic that is used for treatment of hypertension and swelling caused by heart failure and other conditions.
Common Use
Frumil is a combination diuretic—in simple terms, a “water pill” that helps the body get rid of excess fluid. It contains two active ingredients that work together: amiloride (a potassium-sparing diuretic) and furosemide (a loop diuretic). Furosemide helps the kidneys remove excess water and salt from the body, while amiloride helps prevent the loss of too much potassium, which is an important mineral for heart and muscle function.
Frumil is used to treat several conditions where the body retains too much fluid:
High blood pressure (hypertension) – by reducing excess fluid, it helps lower blood pressure and reduces the workload on the heart
Congestive heart failure – a condition where the heart does not pump blood effectively, leading to fluid buildup in the lungs, legs, and other tissues
Edematous syndrome of various origins – swelling caused by fluid retention in conditions such as:
Chronic heart failure
Nephrotic syndrome – a kidney disorder that causes the body to excrete too much protein and retain fluid
Cirrhosis of the liver – scarring of the liver that can lead to fluid buildup in the abdomen (ascites) and legs
Dosage and Direction
If you are being treated for high blood pressure, it is important to understand that Frumil does not cure hypertension—it helps keep it under control. This means you will need to take it consistently, often for several weeks or longer, to maintain the benefits. Even if you feel well, do not stop taking it without speaking to your doctor first.
Take Frumil exactly as prescribed by your doctor, usually once daily in the morning to avoid having to urinate during the night.
Precautions
Before taking Frumil, let your doctor know if you have:
Diabetes – because diuretics can affect blood sugar levels
Kidney disease or liver disease
A history of drug allergies
While taking Frumil, be mindful of the following:
Avoid large amounts of foods high in potassium, such as bananas, baked potatoes, cantaloupe, avocado, raisins, and cooked spinach. Because Frumil contains a potassium-sparing diuretic, eating too many high-potassium foods can cause potassium levels to become too high.
Alcohol may increase the effects of this medication and some of its side effects, such as dizziness. It is best to limit or avoid alcohol while taking Frumil.
Do not stand up too quickly from a lying or sitting position. Sit for a few minutes before rising to avoid feeling dizzy or lightheaded.
Frumil helps the body retain potassium, so there is generally no need to use salt substitutes (which often contain potassium) unless your doctor advises otherwise.
If you are pregnant or planning to become pregnant, inform your doctor before taking this medication.
Contraindications
Frumil should not be used in patients with:
Hypersensitivity (allergy) to amiloride, furosemide, or any other ingredients in the medication
Hyperpotassemia (high potassium levels in the blood)
Severe kidney disorders (such as anuria, where the body stops producing urine)
Hepatic coma (a severe complication of liver disease affecting brain function)
Possible Side Effects
Common side effects of Frumil may include:
Dizziness
Vision changes
Dehydration (dry mouth) or thirst
Nausea and vomiting
Headache
Hyperpotassemia (high potassium levels) – symptoms may include muscle weakness, fatigue, or irregular heartbeat
Seek medical attention if you experience any of the following:
Muscle cramps
Irregular heartbeat
Tingling in the hands or feet
Unusual fatigue or weakness
Difficulty breathing
Difficulty urinating
Drug Interactions
Frumil may interact with a number of other medications. It is important to tell your doctor about all medications you are taking, including:
ACE inhibitors (such as lisinopril, ramipril) – used for high blood pressure and heart failure
Barbiturates (such as phenobarbital) – used for seizures or anxiety
Cholestyramine – used to lower cholesterol
Corticosteroids (such as prednisone) – used for inflammation
Insulin and other diabetes medications – diuretics can affect blood sugar levels
Cyclosporine – used after organ transplants
Muscle relaxants such as tubocurarine
Nonsteroidal anti-inflammatory drugs (NSAIDs) – such as ibuprofen or naproxen, which can reduce the effectiveness of diuretics
Other high blood pressure medications – combining them may cause blood pressure to drop too low
Missed Dose
If you miss a dose, take it as soon as you remember—unless it is almost time for your next dose. If that is the case, skip the missed dose and continue with your regular schedule. Do not take a double dose to make up for a missed one.
Overdose
If you think you have taken too much Frumil, seek emergency medical attention immediately. There are no specific signs of overdose for this medication other than dehydration, but taking too much can cause excessive fluid loss, low blood pressure, and electrolyte imbalances.
Storage
Store Frumil at room temperature between 15–30°C (59–86°F). Keep it away from moisture and direct sunlight. Always keep the medication out of reach of children.
