Aldara (Imiquimod)

Aldara is used to treat precancerous skin lesions. It stimulates human immune system.

Common Use

Aldara contains the active ingredient imiquimod, and it is used to treat two main conditions: actinic keratosis (precancerous skin lesions caused by sun damage) and genital warts (caused by the human papilloma virus, or HPV). In simple terms, Aldara works by stimulating your body’s own immune system to fight off the abnormal skin cells or the virus responsible for the warts. Instead of attacking the problem directly, it helps your natural defences do the job themselves.

Actinic keratosis are rough, scaly patches on the skin that develop after years of sun exposure. If left untreated, some of these patches can turn into a type of skin cancer, which is why treating them early is important. Genital warts are small growths in the genital area caused by HPV; while they are usually not dangerous, they can be uncomfortable and contagious.

Dosage and Direction

Wash your hands thoroughly before handling the cream. Apply a thin layer of Aldara to the affected area—do not rub it in heavily, just gently. The dosing schedule depends on what you are treating:

For genital warts: Apply the cream three times a week (for example, Monday, Wednesday, and Friday). Leave it on for 6 to 10 hours, then wash it off with mild soap and water. Do not use tight bandages that would block air from reaching the skin. Treatment usually continues until the warts are completely gone, or for up to 16 weeks.

For actinic keratosis: Apply the cream once daily for two days a week (for example, Monday and Thursday). Leave it on for about 8 hours, then wash it off. Treatment continues for 16 weeks.

Avoid getting the cream in your eyes, nose, mouth, or any open wounds. If contact happens, rinse thoroughly with water.

Precautions

While you are using Aldara, it is best to avoid sexual contact, especially if you are treating genital warts. The medication is known to weaken latex rubber products such as condoms, diaphragms, and cervical caps, so they may not provide reliable protection against pregnancy or sexually transmitted infections.

Do not use other skin products—such as moisturisers, lotions, or cosmetics—on the same area while you are being treated, unless your doctor tells you otherwise. If you develop flu-like symptoms (fever, chills, muscle aches) or severe skin irritation, let your doctor know. You may need to take a break from the treatment or have your dosing adjusted.

Contraindications

Do not use Aldara if you are pregnant or breastfeeding, unless your doctor specifically advises it. You should also avoid it if you are allergic to imiquimod or any of the other ingredients in the cream. If you’re unsure, it’s always a good idea to have a quick chat with your pharmacist.

Possible Side Effects

The most common side effects occur on the skin where the cream is applied. These may include:

Redness

Swelling

Itching or burning

Sores, blistering, or ulceration

Skin peeling, scabbing, or crusting

Changes in skin colour (usually temporary)

These reactions are a sign that your immune system is responding to the treatment, but if they become severe or very uncomfortable, speak with your doctor. They may recommend taking a short break from the medication.

Drug Interactions

Because Aldara is applied to the skin and is not absorbed into the body in significant amounts, it is not likely to interact with other medications you take by mouth. However, it’s still good practice to let your doctor know about all the products you are using on your skin, as other topical treatments may cause additional irritation.

Missed Dose

If you miss a dose, apply it as soon as you remember. However, if it is almost time for your next scheduled application, simply skip the missed one and continue with your regular routine. Do not apply extra cream to make up for a missed dose.

Overdose

If you apply too much Aldara, you may increase the risk of severe local skin reactions such as intense redness, swelling, or blistering. There may also be a greater chance of systemic effects (feeling generally unwell). If you suspect an overdose, wash off the cream immediately and contact your doctor.

Storage

Store Aldara in a dry place away from direct sunlight, at room temperature between 15–30°C (59–86°F). Keep the tube tightly closed when not in use, and always keep it out of reach of children.
